Community-based services

Community and outreach mental health and wellbeing support for people with serious mental illness or severe and ongoing mental health difficulties.

What we do

We support adults who are experiencing serious mental health illness or severe and ongoing mental health difficulties.

We provide:

  • Personalised, one-to-one support
  • Connect you with local resources, activities and groups
  • Empower you to enhance your independence and mental wellbeing

We make initial contact within 48 hours to arrange a face-to-face assessment, with the possibility of further follow-up appointments to develop a care plan.

Who for

Community Wellbeing Connectors

Our Community Wellbeing Connectors work with those who have ongoing and complex severe mental illness. This service is available to those transitioning from Community Mental Health Teams to Primary Care Mental Health, and those who may benefit from longer-term support.

This service runs at Leeds Student Medical Practice (LSMP), The Light Surgery, and Middleton and Hunslet Primary Care Networks.
