MindMate Single Point of Access (SPA)
Leeds-based service that that triages and directs children and young people to the right emotional wellbeing or mental health support.

What we do
Our MindMate SPA service acts as an entry point to mental health services, offering pathways for urgent help, therapy, and other specialised support.
We help to connect individuals and families to appropriate resources, preventing delays by assessing needs and linking to services such as crisis and neurodevelopmental support.

Which services can you refer me/my child to?
- Early help intervention within school
- School cluster services, such as families or one-to-one work
- ASPIRE who provide early intervention for those aged 14+ with signs of psychosis
- Forward Leeds Young Person’s Drug and Alcohol Service
- Child and Adolescent Mental Health Service (CAMHS)

Who can make a referral?
Young people
Those aged 13 to 17, and parents and carers of children aged 5 to 7.
GP and NHS healthcare professionals
Refer using the guidance and forms on this page.
Social workers
Refer directly if you are unsure whether standard council‑provided services are the right option for the child or young person.
School staff
Refer to the service using the guidance distributed to schools.
School nurses
Continue to use the Emotional Health and Wellbeing Pathway. You can make a direct referral if there has been no improvement in the young person’s wellbeing, or if their needs are beyond the scope of the pathway, for example where autism is suspected.
College staff
Refer directly using the guidance distributed to colleges.

How to refer
If you are aged 13 to 17 and registered with a Leeds GP, you can ask us for help in finding the right support service for you. If you’re under 13, you’ll need an adult to fill out this form with you.

What if I need an urgent referral?
Urgent referrals may be required when a young person is rapidly losing weight or having thoughts to seriously hurt themselves.
You can contact your GP for an urgent appointment, who can assess the physical and mental health issues and make an urgent referral if needed.
What happens after referral?
Once we receive your referral, we will check whether there is enough information to identify the right service. If more information is needed, we may contact the young person, their family, school or other professionals.
We will then send a letter to the young person and their parent or carer, and pass the referral to the most appropriate service.
We will usually involve both the young person and their parent or carer throughout the process. If you are aged 13 or over and do not want your family to be informed, we will respect this.
