What we do

  • Short term evidence-based (proven to work) mental health support for children and young people
  • Support and training to schools to develop their whole school approach to emotional wellbeing.
  • Mental health advice and liaison with services for school staff about pupils. There is a wealth of mental health advice and information on the Open Minds Calderdale website

Calderdale Mental Health Support Team Request For Support Form

This form is for completion by MHST school staff following discussion with an MHST staff member about the potential suitability of young person for support. The print version can also be used and handed to the MHST staff member in School.

Urgent cases should not be referred using this form – see our Open Minds (CAMHS) website for steps to follow if urgent support is required.

How to refer

Referrals are made by trained members of school staff who act as wellbeing leads. They refer directly to Mental Health Support Team practitioners who are based within school.
